Funded by the Foundation for Developmental Disabilities, which supports individuals with intellectual and developmental disabilities through grants, scholarships, and advocacy, Access Point 360 is proudly powered by Raízes, a nonprofit uplifting underserved communities through vital programs and resources.

System Navigation Services
Personalized guidance through Medi-Cal, IHSS, CalFresh, SSDI, and other essential programs—with a specialized focus on supporting individuals with intellectual and developmental disabilities and their families.
Warm Handoffs & Referrals
Direct connections to primary care, specialty services, behavioral health providers, and disability-specific resources to ensure seamless access to comprehensive care.
Support Groups
Safe, inclusive spaces for families navigating autism and other developmental disabilities—fostering community, healing, and shared understanding.
