Case management is a collaborative process where our trained staff work one-on-one with participants to: 

  • Assess needs and goals 
  • Develop personalized care plans 
  • Coordinate access to services 
  • Monitor progress and provide follow-up

  • Enrollment in Medi-Cal, IHSS, CalFresh, and SSDI 
  • Referrals to primary care, behavioral health, and specialty providers 
  • Support with housing, transportation, and employment resources 
  • Connection to autism support groups and family services 
  • Crisis intervention and advocacy
